Attaching an M-371A Form (Permit Inspection Costs)

An M-371A form can only be attached after Creating a Reimbursable Record for the permit. An M-371A form must be saved as a PDF file that is accessible by the user attaching it to the reimbursable record.

To attach an M-371A form to a reimbursable record:

1.      Navigate to the Manage Reimbursements Screen for the selected reimbursable record.

2.      Click the [Browse] button to the right of the M-371A Attachment field.

3.      In the Choose File to Upload dialogue box, navigate to the correct directory, select the appropriate PDF file, and then click the [Open] button.

4.      Click the [Attach] button to attach the selected M-371A PDF file to the reimbursable record.

5.      Continue on to Ready the Reimbursable for Authorization or click the [Save & Exit] button to return to the Inspections & Closeout Screen .

After an M-371A form has been attached:

·        The reimbursable record remains in Draft status.

·        The [Ready for Authorization] button appears, and an authorized PennDOT user can Ready the Reimbursable for Authorization.

·        An authorized PennDOT user can Delete a Reimbursable Record from the permit (while it is in Draft status).
