Populated when permit is generated. Cannot be modified after entry except by EPS Administrator.

Supplement Permit No.

C

Displays the supplement type selected from the Create Supplement button dropdown list. In the case of a Time Extension Supplement, it displays Time Extension.’

Supplement Expiration Date

C

This is the Date the new supplement being created will expire. The Supplement Expiration Date options include 6 Months, 12 Months, and Other.

  • Selecting 12 Months will change the New Expiration Date field to display the date one year from the Current Expiration Date.

  • Selecting 6 Months will change the New Expiration Date field to display the date six months from the Current Expiration Date.

  • Selecting Other will display/enable a new field from which the user can enter or select a specific expiration date. Selecting Other removes the New Expiration Date field.

A selection must be made in order to save and/or move away from this screen.

Issue Date

C

Available to responders with Update rights to this application. It cannot be modified after selection.

This field becomes mandatory when the application status changes to Permit Issued.

Close-Out Date

Available to responders with Update rights to this application.

Expiration Date

C

The permit Expiration Date is determined based on the permit Issue Date and the Expiration Date option chosen by the Responder on the Permit Information screen. The Expiration Date options include 6 Months, 12 Months, and Other.

If either the 6 or 12 Months option is selected for the expiration date, the actual expiration date is not displayed; rather, the selected option (6 months or 12 months) is displayed in the Expiration Date field. The actual expiration date is calculated based on the Issue Date, saved in the database, and displayed on the Permit form.

In order for the applicant to receive the full permit life (6 months or 12 months) the Responder should select the Expiration Date at the same time the Received Recording Notice Date is entered on the Permit Information screen.

If Other is selected as the expiration date, the actual expiration date must be manually entered by the Responder; it is displayed on the Permit Information screen, and is saved in the database for display on the Permit form.

Available to responders with Update rights to this application; cannot be modified. Defaults to 12 months; can select 6 months; can select Other to enter a unique date.

This field becomes mandatory when the application status changes to Permit Issued.

Received Recording Notice Date

Available to responders with Update rights to this application. This is entered when PennDOT receives the signed and notarized recording document from the Applicant.

Permit Recorded Date

C

Available to responders with Update rights to this application. This is entered when PennDOT receives the recording document back from the county.

Notification Date of Ready for Closeout

C

Date that PennDOT receives notice that work is completed and the permit is ready to closeout.

Security Information section

Security Type

Dropdown indicating whether security is Bond or Letter of Credit.

Security Number

C

The bond issuer or letter of credit number.

Issuer Name

C

The name of the issuer of the permit security.

Expiration

C

The expiration date of the permit security. This date is used to determine the Blanket Security Amount on the Fee Information screen. An expiration date earlier than the current date indicates that this bond or letter of credit is no longer in-force.

Security Amount

C

The dollar value of bond or letter of credit. This amount is used to determine the Blanket Security Amount on the Fee Information screen. Blanket Security Amount is the total of all in-force bond value and letter of credit value for all security agreements associated with the customer account for the applicant on the application.

Security Attachments section

File Name

C

File name of the document selected from the Browse window. New documents are incorporated into the list in numeric order.

Description

C

Description of document name that is selected form the Browse window.

Attached By

C

Name of the user who attached the document. Populated by the system when the attachment is made.

Date

C

Date the document was attached. Populated by the system when the attachment is made.
