The Create Supplement screen is used to create supplement applications for time extension, change in work, fee change, and permittee name changes. These supplements can be entered for highway occupancy and traffic signal permits (including minimum use driveways) and/or bridge occupancy licenses. Both internal and external ePermitting users can create and submit supplement applications for issued permits that they were associated through assignment to the Applicant Team or Review Team.

The Create Supplement screen is dynamic in that the type of supplement selected from the Create Supplement button on the Applications Portal screen determines which fields are available on the Create Supplement screen. For all types of supplements, the Create Supplement displays the permit application number and permit number for the permit to which the supplement is associated.

Note: A supplement application cannot be submitted if the original utility permit is more than 30 days expired. An error message displays:

“A supplement request can no longer be submitted since it is more than 30 days beyond the expiration of the permit.  An application for a new permit may be submitted.”
